During independent reading, I conduct reading conference as part of assessing their progress. I like to spend about 5 minutes with a student during each conference. During which I take a running record, write down student strategies, take note if the student chose a just right book, and what I my teaching point is for that conference.
After many attempts to create a form that works for my conferences, I have finally created this form that seems to do the trick.
This form includes space for three conferences on each page so I can refer back to my previous conference when I'm ready to conduct a new conference. I have a form for each student on a clip board and rotate the pages as I move from student to student.
Tip: Print front and back to keep 6 conference notes at a time on one page for every student.
