In this file you will find guides for the teacher with focus questions, space for feedback and an easy-to-use rubric. Corresponding student forms require them to prepare before the conference and to record feedback to use later. Finally, there is a file of forms that will allow you to track student progress. Each guide is completely editable, so you can tweak them as needed.
