This reading record has been designed to use to record decoding and reading continuum progress.
These pages should be copied back to back, with multiple copies of pages 3-4 if you want to use one record for the whole year. One double sided page has enough space record eight conferences.
I use for small guided reading groups, individual conferences and they are small enough that if I want to grab a stack to take home they don't occupy my while briefcase.
The front and back covers provide a place to record year long progress as well as modified version of Stephanie Harvey's and Harvey Daniels Comprehension Continuum from Comprehension Going Forward (2009).
The inside provide a quick check off of reading strategies your readers are using (or not using) and an area to check off observations such as Sight Words, Using but Confusing (a.k.a Miscues for Meaning, Syntax and Visual), Just Right Book, Fluency, Connections, Comprehension and an area to record anecdotal notes.
The difference between this and the K-1 is that there I have removed the option to track early phonemic awareness skills (id letters, sounds, rhyming, blending, etc. which can be found in the K-1 version).
