This is a form I created to discuss a student's independent reading with him/her. I like to conference with them to gauge their progress and understanding of the books they read independently. This becomes a record that I can use to show concrete feedback to the parents during conferences. I can fill out this form in a matter of 2-3 minute conference. The true beauty of it is that I've incorporated a rating system for comprehension, strategy use, and fluency that I just have to circle on a scale of 1-5. This both speads up my conference to free up more class time, but allows parents to see how their son/daughter is measuring up to the average student.
