I use this form when conferring individually with my students. To streamline note taking, there are "check boxes" to make it simple, fast and efficient. I believe all great conferences start with listening in to the student to hear them read, and giving them a compliment on their reading. This helps to build a reading relationship with your student, and build confidence. There is a section for the type of book the child is reading ( CH for chapter book, P for picture book, F for fiction and N for nonfiction) These are meant to be quickly circled during conference. There are also spots to circle after giving comprehension questions. I circle these if they got my questions correct, and leave blank or x out if they miss the question. Prompts are as follows: C: character, S: setting, B:beginning, M:middle, E:end.
