This English Language Arts (ELA) assessment record for common core for the fourth grade is what I keep as essentially a student gradebook. I am a "techie" person but I have found nothing better over the years to both keep track of my students and to use to talk with them on the fly about their goals and scores. I print one out for each student at the beginning of the year and write scores in with pencil as we go through the year. At the end of the year, I make a copy for students to take home as a more meaningful record of their progress than the official report card.

All of the Common Core reading standards are listed, grouped into three categories - Foundational Skills (Phonics/Fluency), the main skills, and then reading grade level material independently and proficiently. Next to them are columns for entering scores or other data. Below each of the three groups is also a set of assessment score areas - one for fluency (DIBELS), one for the main set (DIBELS, Daze, and practice state tests), and one for reading grade level material (Accelerated Reader points and goals). All of this is on a one-page sheet for easy access.

Since we are standards-based, I don't have to keep records of every set of daily or weekly reading work, only the benchmark data. This gives me a constant, current view of each of my students and is what I use when talking with students in class, parents at conferences or with colleagues discussing placement. Because it shows the benchmark goals along with the student scores students, parents, teachers and administrators can clearly and easily see how the student's scores compare to the standard. When it comes time to fill out report cards or goal sheets, this is what I use.
