This form can be used by teachers to track the progress of students' reading fluency skills. What's great about it is a teacher can use as few or as many of the options on the log, as they deem appropriate for their teaching needs.
With this form, a teacher can:
Tracking students' time and page numbers
Track number of times a story was read before it was mastered
Distinguish between un-leveled and leveled text, with space to add level
Mark all misread words - and what they read it as
Mark what kind of error student made: teacher told, skipped, sounded out word (syllable division), read as
Note/rate students' comprehension, appropriate inflection, and fluency.
This form was created with students with Dyslexia in mind. The form enables a teacher to recognize any patterns in students' errors.
I use this form in my own practice when tracking the progress of my students that I work with individually, and I print out the log double-sided.
