I use this as my first lesson for reader's workshop during the school year. Students must look at their reading weaknesses in order to find how they will grow as a reader during the year, and then brainstorm how they can reach that goal.
It is something that I can check in with them while conferencing, and if students meet their goal, I am able to teach them the importance of creating even loftier or completely different reading goals to continue growing their reading life.
I have students complete two copies of their goal and action plan. One copy goes on a bulletin board, while the other is glued to the first page of their reading response journal.
