It is difficult as a reading teacher to ensure that students maintain their individualized reading intervention plan upon returning to school from summer break at the beginning of the school year. The Reading Transition Sheet can be passed on to next year’s classroom teachers by reading intervention specialists to inform them of their student’s reading intervention plan. It can also be copied for reading teacher reference come fall. Classroom teachers would also benefit from providing this sheet to their student's previous intervention provider to fill out for them.
Included in this easy to fill out form is strengths and weaknesses of the student, progress monitoring tools used in the previous year and interventions that have previously worked for the student. There is a check off list to show what kind of action and plan will take place in September to ensure that the student is receiving appropriate services in accordance with RTI. Classroom teachers have told me that they found it helpful to them. I hope you find it valuable as well.
