I wish I had this when I was teaching sixth grade reading. This might be my favorite thing ever. You can use these standards based activities with ANY novel. They can be used for a read aloud, like Freak the Mighty, or students can work on independent skills using their own book of interest. They can be used multiple times, as the skills continue to build through the school year. Each standard has anchor charts for guiding lessons, Lesson planning guide with the standard "unpacked for understanding," student activities aligned to each part of the standard, and CCSS aligned assessments to show mastery by standard. Activities can be used for small groups, and there is a small group planner to keep track of some of the details. Students track their own understanding throughout and once a standard is mastered celebrate success with "ticket to mastery."
This has everything. You provide the books of your choice, or the choices of your kids.
