Are you sick of post-it notes falling out of your student's books? What do you do with all the post-its when they finish their books? Their notes and thinking should be treasured! I developed this template as a simple way for students to take notes while reading. This works with any reading program. I've used it during Read to Self time of Daily 5, and with Lucy Calkin's Reading Workshop.
When we are focusing on character traits we use this form. There are 3 different templates with a variety of sentence frames to study characters as well as compare and contrast characters. If you want a version you can edit with your own sentence frames, just let me know!
In my classroom, reluctant note takers get a quick highlighted box or two (or three!) from me as a motivator to record their thoughts. All of my students have time at the end of their independent reading to reflect on their notes and choose their most valuable treasure to share with me or their book clubs.
I find that scanning their notes guides my Reading Conferences with them so much easier than trying to flip through their mess of post-its!
