Welcome to the Reading Passport Book Report. Book reports don’t have to be a chore. In this book report, students can create a “reading passport” for a specific book that they have read.
This worksheet will work well with grades from 2nd grade to fifth grade. Students will need to read a book before filling out the book report. This book report has areas for a drawing, basic biographical information, a summary of the book, and an opinion why the reader likes the book. It makes a great substitute worksheet, book report booklet, and a wonderful bulletin board.
