Chart for Guided Reading, with guided reading levels compared with other leveling systems and grade level, a place to record date, title of book/assessment, pass or fail retell/comprehension checks, and a key of symbols to use. Draw a line vertically down the chart once you find their baseline and before you start instruction, or when you change reading strategies. From the baseline, draw a dotted line to the goal at the end of the year. As you go along, connect the level with a solid line to compare against the goal line. I've used this chart for many years. A great visual to show parents and administrators.
