I use this reading log with my third-grade students, but I think that it is suitable for any student in grades 2-6. This log is intended for students to keep track of both in-school and at-home independent reading.
There are columns for the date of reading; the title of the book; the genre of the book; the pages read; the minutes read; a 1-5 rating; and an assessment of whether the book was too easy (E), just right (JR), or too hard (H). At the bottom of the log is a place for a parent signature and the date signed. This log will track a student's independent reading for a seven-day week (weekdays and weekend).
In this file, I have also included a list of popular book genres. I usually copy this list onto the back of the reading log. I have found that it assists my students with identifying the genres of their books.
This reading log has been a good motivator for my students, and they enjoy having a clear record of the books that they are reading.
