This 2-sided worksheet is the perfect way to get intermediate and middle school students thinking about their reading goal for the coming year. One side prompts students to write down the number of books they'd like to read in 2016 (and how many that works out to each month), ways in which they will keep track of the books they read, and a checklist of other possible goals. The back side of the worksheet is a reading log that includes spaces for the date a book was started/ended, its title, the author, and the number of pages. Once students fill out the worksheet you can have them write their main goal on the "My Read-olution" sheet (free on my TPT site)
and post it in the classroom as a friendly reminder.
