Start off Reader's Workshop & Independent Reading time with a resource that helps keep your students organized. It is aligned to 5th grade Common Core ELA and Reading Standards, but can be adjusted for lower or higher grades.
The reading notebook can easily be put into a binder for the students to collect all of their activities throughout the year. The sections include:

Although some of the pages are aligned with Accelerated Reader, I made duplicates that do not have the AR language for people who don't use Accelerated Reader.
Because it is in a notebook form, the students can add to the pages through out the year. You can include your own notes, activities, and assessments. It's an excellent resource during conferences to show the student's progress in reading.
