Easy to use Reading Responses/Book Report forms are here for you! They are easy to assemble in a Reading Notebook that students keep all year long! You get 11 different reading response sheets. Check out the thumbnails to get an idea!
I have found that it works well to have the students set up Reading Notebooks. The reading log (included) can be glued into the first pages of the notebook.
Included also is a Reading Log for students to track their reading progress. The students can use the reading log to log the books they read. Included here is a READ 35! log. It gives the students a goal to read 35 books over the school year in a variety of text types. My log includes: fiction, non-fiction, poetry, picture books, and student choice books. You could edit this log for the amount of books and text types you want to use or forgo the use of the log portion altogether if it does not work for your purpose.
Finally, the bulk of this resource is 11 different reading responses. These responses include: fiction, nonfiction, poetry, mystery, world records, comics/cartoons, biography, etc. (Check out the Table of Contents). Once a student finishes a book, they can log the title in their Reading Log and complete a reading response. The reading response then gets glued into their reading notebook as proof of understanding what they read.
My students love their reading notebooks! We have reading time every day where they are either reading or working on a response. Remember: Research shows the more they read, the better readers they become!
