Get to know your students and start assessing them as a reader on the first day or first week of school!
This quick survey offers students a chance to share who they are as a reader as well as their reading preferences. You can use this to refer back during reading conferences, parent conferences, and during goal setting. Show your students that you want to get to know them on an individual basis and start building relationships from the first day of school.
Two versions are included:
The first version (both with and without borders) has 14 questions in the reading interest survey. The questions fit on 2 pages and allow for you to print back to back to save paper.
The second version (both with and without borders) is a bit longer with 21 questions on 3 pages. This version goes a bit deeper to learn about the reading history of students.
Questions include asking students what type of books they do enjoy so you can start to plan for book talks and identifying books your individual students will enjoy.
ALSO INCLUDED-JUST ADDED-Reader's Profile Page and Reading Conference Page. Use the information you gather on the survey to take notes on what your student's like to start making book recommendations. (I added this page because I kept losing the post-its I'd make after reviewing the surveys). The reading conference page lists strengths and goals and allows you to keep anecdotal records on individual students when you confer with them.
