My students do Reading Workshop 4 days per week. They needed an organized way to write their responses that could easily be turned in each week to be graded. This Word document is the form my classroom uses. It has 4 spaces, labeled for Monday, Tuesday, Wednesday, and Friday, that could easily be changed to fit your classroom schedule.
