Included in this EDITABLE BUNDLE are my Reading, Writing and Maths anecdotal notes, records and observations, all of which are available seperatly in my store.

READING: ANECDOTAL NOTES, RECORDS AND OBSERVATIONS - ASSESSMENT FOLDER

This is a modified version of my own Anecdotal Reading Notes Folder. Head over to my blog - missmelsclass.blogsport.com.au - to check out how I use mine and how you may like to use yours!
I have used these documents for each Semester, as well as for the whole year, depending on my class needs. I print all the pages I need, the get it bound, howver you can store it however you like!

Included in this .zip file include:
*your choice of 10 different pre-made cover pages (2 each of blue, purple, green, orange or red)
*5 editable cover plate templates
*an editable 'Running Records' record page, where you can type in your students' names, then print off for your records, filling it in once a month OR fill in the template on your computer.
*2 'Student Record' pages (designed to be printed back to back) where you can enter your student's name as well as the period of observations (Term 1, Semester 1 etc). Each of these pages has space for 5 reading observations per term/semester/quarter etc (your choice of time frame). Simply enter the student's name and the time period on the documnet, print it, then record your notes and observations when listening to them reading/during reading conference
*2 pages of guided reading group notes templates, designed to be used each week of the term/quarter/semester. 2 versions of this template are included, both are editable for your needs; the first version has a set font, whilst the second version is completly editable. On both of these templates you can insert your guided reading groups name/picture/colour etc

*2 editable pages for records/analysis of student writing samples. Simply type the name of your student into the document and print it, recording your notes on the template. Alternatively save it and type directly into the documnet. There is space on this template for the analysis/note taking on 3 writing samples (designed to be used per Semester).
*2 editable versions of my 'Cross Check' template. Each template has enough space for 25 students. The first version has a set font, whilst the second version is completly editable. Type your students names into the top left corner of each box and print a bunch off. Have them on hand (or collate them into a folder/book like I do) and record whole class/group specific observations, such as your grammar/spelling focus for the week. Write this under the "What I'm Looking For" section at the top, and and record your notes in the releavnt student boxes as necessary. Can be used for whole class or only certain groups/students. (More info on how I use cross-checks is on my blog).
*2 editable versions of my Narrative text type/genre checklist. Each checklist has enough space for 26 students. The first version has a set font, the second version is completly editable. Simply enter your students' names down the left side and then, when analysing a writing sample of this text type, tick, dot or cross the text features accordingly. This is a very simple way to understand the features of texts students may need extra support with.
**2 editable versions of my Persuasive (exposition) text type/genre checklist. Each checklist has enough space for 26 students. The first version has a set font, the second version is completly editable. Simply enter your students' names down the left side and then, when analysing a writing sample of this text type, tick, dot or cross the text features accordingly. This is a very simple way to understand the features of texts students may need extra support with.
*2 editable versions of my Recount text type/genre checklist. Each checklist has enough space for 26 students. The first version has a set font, the second version is completly editable. Simply enter your students' names down the left side and then, when analysing a writing sample of this text type, tick, dot or cross the text features accordingly. This is a very simple way to understand the features of texts students may need extra support with.
*2 editable versions of my Procedure text type/genre checklist. Each checklist has enough space for 26 students. The first version has a set font, the second version is completly editable. Simply enter your students' names down the left side and then, when analysing a writing sample of this text type, tick, dot or cross the text features accordingly. This is a very simple way to understand the features of texts students may need extra support with.
*3 editable versions of my Punctation Checklist. Each checklist has enough space for 26 students. The first version has the 12 most common punctuation symbols across the top of the table, then space for your students' names down the left side. Enter your students names and print off, recording when they can 100% of the time use that punctuation symbol correctly and accurately. Could be used 1 for each term to show student progress. The second version is the same, except replaces the symbols for the written names, EG ? becomes question mark. (As this is editable, I have written "full stop" for a period, however you can change this if necessary). The third copy of the checklist is suited for the VCOP/BIG Write program.
*4 editable versions of a spelling results tracker. Each spelling test results template has enough space for 26 students. The first version has space to record spelling test results for 42 school weeks, in a pre-test/post test situation. The second version has space to record spelling test results for 11 weeks, as it is designed to be used each Term. This is also designed to be used for a pre-test/post-test situation. The third version is again split into Terms (4 pages in total), however there is only one space to record your spelling results; this is ideal if you only complete ONE spelling test a week, rather than 2 (pre and post). The fourth version is again for use if you only administer ONE spellling test a week, but this template has space for records across 42 weeks and is not divided into terms.
*Included is a SOUND WAVES SPELLING PROGRAM results recording sheet. This template is designed to be used in conjunction ONLY with this spelling program. This template has space for the 36 SOUND WAVES SPELLING PROGRAM units int the pre-test/post-test format. Check out my blog for how I use this template.

Simply print the required amount you need of these blank templates, collate them, and then record your observations as you work with your maths groupings Alternatively, you could type students' names in then print the template off, or keep the template entirely digital. Each version also indicates where you could put your maths group animal/number/colour/symbol etc, to keep easy track of your notes. Each template has enough space for 6 students per group. Simply type in your students' names in the top left corner of each box (space for 25 students), print off the required amount and record your notes/observations.
