This is a simple, one page (front and back) reading and interest survey that I give my students at the beginning of every school year. I mix in questions about reading with questions about their general interests (movies, music, candy, hobbies, etc.), and it can be completed in 10-15 minutes. I use the results of the survey to determine everything from future class activities, to which cartoon characters and celebrities I may feature in our lessons. It's also fun to pull out during parent-teacher conferences to see what parents and guardians think. This year I adjusted it to include their preferences for digital resources.
