I use this in conjunction with Fountas and Pinnell's "First 20 Days of Independent Reading." The students write me a letter about their books in a journal and then I write them letters back. The students turn in their journals on different days so that I don't have to grade them all at once. Just write in their names on the sheet so that they know what day they need to turn in their reading journals.
I have included six different charts in different colors. The final chart is black and white.
