This file contains an employment application google form that you can assign to students to collect data on personal information. This form asks for name, address, phone, city, state, zip code, position wanted, e-mail address, last school attended. The form changes the location of boxes every time to ensure skills generalization.
The nice thing about using google forms is that students can use extensions, such as Readloud (reads prompts to them) and VoiceinVoice (speech to text) to complete their work. You and your students can watch videos that explain how to use them on Youtube. My kids picked them up extremely quickly.
I assign this to my Life Skills students daily. Each day, the information is collected, and can be downloaded into a spreadsheet. I can then highlight errors and look for trends over time. These can be used on an iPad, Chromebook, or desktop. If you use Google Classroom, they can easily by assigned as daily work.
