To prepare: You can either print out all of the pages and put them in a binder or print out the select pages that you need for individual students when you need them.

To use: Select the pages that you would like to use. Read the directions to the student and have the student follow them. You can either write their responses on the therapist copy sheets or you can use the data sheet (found on page 141) to record responses. Page 141 also includes percentages for easy conversion as well as an area for any notes that you may feel like taking while assessing the student.

*Suggestion: You may want to use one sheet for obtaining baseline data and the other sheet for follow-up to determine progress that has been made.

*Slides 146 or 147 can be used as the cover of this document. There are two backgrounds for the cover. Slide 148 can be used for the spine of a binder. There are two smaller strips and two larger strips, each in two colors.
