This reciprocal reading chart requires students to make connections (text to self, text and the world; develop HOT (higher order thinking) questions; summarize (who, what, where, etc.); and making a prediction. Students also have any opportunity to write down any new vocab words and to identify the author's purpose, depending on the genre. There is a place for them to track their pages, both their goal and the page they ended on.
Also included is a group and self-accountability rubric that students can use each time they read. They can give both themselves and their groups a grade, and there is a conversion chart at the bottom so they can see their self given score in a %. There is also a short area for them to give themselves and their group glows and grows. I use this chart and rubric religiously with my students and its a great way of keeping them on task and accountable.
