I get a lot of requests for referrals throughout the year and it is important to stay on top of them due to the deadlines placed upon us by federal and state law. Some are formal requests that require a strict timeline, and others are not, but I like to keep them all organized with one document such as this "Referral Requests". This is an excel document that works as an ongoing list of the student's referred. As the student works their way through the referral process you work your way form left to right in the document--> The first column is noting the date of the referral...the next, student's name, then school and grade. Then, you note your response--will you proceed with an evaluation or not--as well as the date. If so, the next few columns walk you through everything needed: parental consent obtained, the date that consent was received (so you can stay within the timeframe for the eval), what tests you completed and the date, whether teacher and parent input were received, and the date that the Case Conference was scheduled. Finally, the last column is to note the outcome of whether the child qualified or not so you can add the student to your caseload of need be.
