I created this for my 9th grade students who were struggling with taking notes for a research project.
Rather than using index cards, which are easily lost, this template can be photocopied and hole-punched to be placed into a binder, or stuck in a folder, for easy access. The top of each box has room for the student to list the title, author(s), and where they found the source so they can easily find it again when its time to make their reference page. You can easily add more information if you prefer your students to list more information.
This template has room for notes to be divided into two categories: "they say" and "my words." This was key for my students. They would write down good quotes to use and notes in their own words, but then when it was time to write the paper, they would forget what wasn't in their own words already. This template takes care of that. "They say" is for direct quotes and "My Words" is for notes in their own words.
