I use this handout during research reports. The citation information is at the top, notes that are gleaned from the source are in the middle, and focus words/summary is at the bottom.
I find this really helps my middle school students keep their information together. I found notecards caused headaches for everyone involved; no floating, lost notecards with these handouts.
There is a place beside each note for the subtopic the note would fit into - this is assuming that students have made a rough outline of basic topics before starting their research.
