This is the typical form that I use in my student centered class. I have students complete one resource record for each section I assign.
I always assign a read, write, watch and do for each section we are learning. After completing the section, each student fills out this resource record.
The most important part is that they master the objectives, that I always include in their assignments. They need to be specific about which objective they have mastered.
This is a great way to differentiate grading.
