This form works well for keeping track of incidents in the classroom. It is also really helpful for us as teachers to not respond in the heat of the frustration. Instead of me immediately responding to their behavior, I provide them with the incident form. This gives them time to cool down as they are reflecting as well as giving me time to consider what I am going to say. Additionally, the form gives you insight into the situation without making judgments right away. You have an idea of where the student is coming from when you read it. The form can also be used to guide any follow-up conversation that might be had with the student. Additionally, forms are helpful for keeping track of student behavior or for communicating with parents. Aka, documentation. I can testify that Restorative Practices work.
