Topics/terms on both the review sheet and test include: The three types of rocks and their characteristics, the uses of rock, minerals, and the rock cycle.
The review sheet is a 16 question mix of fill-in-the-blank and short answer questions. The test is also 16 questions with matching, fill-in-the-blank, and short answer. The last page (questions 15 & 16) takes more explaining and is more difficult. You may chose to only use that page with more capable students or classes. There is also a blank rock cycle chart with an answer key. This chart could be used for both the review and then the test.
If you already teach the above mentioned content and are happy with the materials you use, this test would likely fit with what you already do. However, if you are looking for a new way to present the material, I have an interactive, PowerPoint with animations and matching sound effects, that follows these papers exactly. It also contains many pictures that demonstrate the characteristics of each type of rock. There are two interactive rock cycle activities included. One is a "click, drag, and place" NOTEBOOK file, the other is within the PowerPoint itself.
