I created these checklists before RtI was all the rage. I use them at the middle school level, but they would work at all levels. These checklists are perfect for creating a paper trail of a student's behavior. They are best used at the beginning of the RtI process to prove that the student need extra services. They are also a communication tool so that parents can see what is happening.
There are 3 checklists: daily behavior, daily academic, weekly
These are editable so that you can input the classes needed and the behaviors you want to target.
