This product is a running record template. With it, you are able to do a running record on any student, using any passage or book. Simply listen to a student read for 100 words. As the student is reading, make a check mark in each box if the student reads the word correctly. If the student makes an error, record the error in the box. Record an "SC" if the student self-corrects.
Tally how many errors the student made vs. how many words the student read correctly. Since this is only out of 100 words, this number (words read correctly) turns into the percent. Check off the student's accuracy level. Record how many errors the student self-corrected vs. how many total errors were made. Monitor the student's fluency. Check off all behaviors that apply to the oral reading. Ask the student to retell or summarize. You many also ask the student questions about what he/she read. Record their level of understanding.
This chart can be used even during a guided reading group by focusing in on one student's reading and fill out the chart!
(Note: If your student's book does not have 100 words, modify the chart. Have the student read 50 words and multiply by 2, etc.)
