WOW...I absolutely love this tool. Each time I teach a new common core math standard, I give my students a pre-test. Based on that information, I am able to group my "guided" math groups accordingly.


This tool is great for recording the results and forming your math groups based upon the data. Each page has the following data fields:
*math standard number
*description of math standard
*four separate ranking fields...Advanced, proficient, needs improvement, and academic warning
*An area for note taking

Keep in mind...this is also a wonderful tool to show evidence of differentiation during evaluation cycles. This tool will make you LOOK GOOD!! :)
