This is just a starting block that I put together for my SLP graduate students. They were able to use it as a portfolio and keep items such as tips/suggestions and other important papers like student observations and supervisor observations, and self-reflections inside. It was a nice way to keep everything together and something they can take with them to future placements. For their first day I listed every student they would see on the “Student Notes” form for them to jot down a few things about each child. I used the “Questions I have” page the most, as I put it on the back of almost every handout/form in an attempt to encourage them to write questions down and ask them.

Lesson Observation Form
- I had the students use this when first observing my therapy sessions. I like this one in particular, because it makes them look at and think about all the different components to a therapy session.
