This is the system that I use for planning sessions and taking daily notes. It is a grid in the form of an excel file and is editable. The following information is housed in the spreadsheet:
- Student name
- Student date of birth
- Student grade
- Student goals
- Dates of sessions
- Abbreviated version of goals
- Planned activities for each goal
- Data for each goal
- Homework/Notes for each date
You can use one spreadsheet per student. You can copy the spreadsheet to additional tabs at the bottom (and rename each tab for each student), or create a different copy of the entire file for each student. On each grid, I type in the student name, DOB, grade, and goals on the top. Then I print out the grid and write in the abbreviated version of each goal (a short phrase such as "vocab in context") and keep all grids in a binder. Before each session, I write in the date and planned activities. During the session, I take data using small tally marks on the grid (or on a separate sheet for articulation goals) and write in the percentage so I can quickly scan the data when I do progress reports. When I fill all the columns on the grid, I just print another. I have also included a blank PDF version if you prefer to print the grid without editing any fields first.
