Superhero STAR! (to go with my classroom theme this year)
I use this constantly, especially the monitoring cards -- the students really buy into those, and review behavior expectations after any lengthy break; it's easy to remember and keeps us all using the same language of expected behavior.
It is my experience that students who have ownership in the classroom will participate more, have less behavior problems, and make more academic growth. To have ownership, students must be accountable to not only themselves but the classroom community.
We use the acronym STAR to focus on successful school and work habits (to use a trendy term - soft skills!)
These 4 areas are also handy for discussing when the community starts to fall apart or is doing really well.
I have included posters, a whole class observation grid, a reflection sheet (to plan how to rejoin the community), individual goal cards (for students who need a visual reminder), and homework passes (for rewards to superior community members or for students who reflected and made choices to improve.)
