These sheets are great for students to reflect and record their data throughout the year for each reading standard.
How I use in my class- I hole punch these sheets, and students place in a 3-prong folder. Each time we take a quiz or assessment on certain standard- students are given back these assessments during a small group data chat. During our chat they record their score on these data sheets and I answer questions they have about the assessment. Students will then reflect on the same sheet what they need to work on for the next quiz date. Students then take home the assessment and correct what they need to and a parent signs their data sheet with score. I usually send home on Wednesdays and they need to returned signed by Friday- but you can create a schedule that works best for you. Once the student has mastered at least 3 quizzes or assessments for that standard- the student can check off in the upper corner they have mastered that standard. I also record on my data board.
