This is a set of 13 assessments I made for the whole year that gives me a better understanding of whether or not my students are learning and applying what they are learning. In the first part they name the sound(s) and what keyword helps remind them of that sound: nest /n/. (They are in order of how introduced in the lessons.) The next section is reading words that contain the sounds from the first part. I tried to use unfamiliar words to see if they really were applying what was learned. The third part usually is sight words. They are also tested in the order they are introduced. You will more than likely see the sight words overlap on more than 1 test. (No sight words on first test or when no new words are introduced) In the corner of the test it will tell you when you can administer that test. If it says lesson 25, don¢¯¿½¯¿½t test until after teaching lesson 24. I use the lines to make notes of what was said if wrong or just check if correct. I make a note whether or not they needed help or if they self-corrected. On the very bottom of the page I make notes of behaviors the students¢¯¿½¯¿½ exhibit.
