This is a paper I used in my classes with great success. On one side it guides students though completing the square on the other side it guides students through solving for x after they have completed the square. This takes into account problems where there is an a>1 in front of the x^2.
I printed off enough color copies for each student in my largest class, then I laminated them all. I didn't have card-stock, so I just printed on two pages and laminated them back to back. My students use a dry erase markers to fill in the boxes for each problem. This also makes going back to check where students had issues, when they get a problem wrong, much easier.
Even students who don't solve the problems on the sheet can use it to double check if they missed any steps.
