Here is a form for students who need or want a reminder of the solutions you have discussed in counseling. For example, in a conflict resolution meeting with 4 students, each student wrote the problem: "working together cooperatively on a group project". As we talked, they wrote down solutions they wanted to try. Ideas they wrote were to take a break and cool off, compromise, be flexible, listen to each other's ideas, share responsibilities, and communicate kindly. During an individual session a student whose volatile feelings often landed him in trouble wrote out solutions for managing his feelings. This can be used in any situation where adjustments in the student's behavior or attitude will help him or her feel better and be more successful. Writing out the solutions seems to help some students remember them. Other students enjoy the visual reminder.
It is a word document, so feel free to edit as you like. Enjoy!:-)
