Here’s a super simple, print and go assessment for your lessons on electric circuits. It meets two of the new NGSS standards for fourth grade:

4-PS3-2. Energy can be transferred from place to place by electric currents, which can then be used locally to produce motion, sound, heat, or light.
4-PS3-4. Apply scientific ideas to solve design problems. (Observe, test, and describe a device that converts energy from one form to another.)

Developed for my own inclusive classroom, this assessment contains scaffolded versions for students who may be unable to draw and write the explanation on their own.
Print on plain copy paper, front to back, and fold. For general education students, just use the first two pages. Substitute page three or four, depending on the level of extra support needed for special needs students. General education students should also be able to draw the path that the electricity takes using a colored pencil or colored marker on their flashlight diagram.

The fun secret ingredient for this performance assessment is a translucent flashlight, making it easy to observe the way in which the switch completes the circuit by joining the contact strips, allowing the energy to flow. May also be used with a standard flashlight.
