A "Scoot" answer sheet for students to show their work and record their answers during "Scoot." Can be used for a wide variety of subjects and questions. I've got some sample Scoot Question cards you can download to use with this answer sheet.
How to "play" Scoot: Have twelve questions cut apart on small pieces of paper (you can cut up worksheets, task cards, test questions, etc.). Place these questions in different parts of the room. You can make this as simple (spreading them out on desks) or difficult (taped to the bottom of chairs and desks) as you think will work for your kiddos. They "scoot" around the room and answer the questions in any order they want, showing their work in the boxes. They can work in partners or individually.
