Our social skills groups are called The Touchdown Club. After every activity we have the students score themselves with either a touchdown or fumble. This has worked really well!
We normally have 2-3 activities beginning with "check-in" (so the kiddos write that under 1st activity box. Then we either read a Julia Cook book or a social story we have and we discuss. After the lesson the kids would write the activity we just did and then score themselves again.
The bottom of the paper has blank lines. If the kids really want to share something they know they can't interrupt or blurt so they write it down and we can talk about what they wrote during the last minute or two of class.
