This is a great resource to use to track your students fluency throughout the year. I have the unit, the week, and the fluency passages name. The green indicates where the student should be during that specific time. This tracker sheet is set up to record individual progress. This is also a great resource/visual to use at parent conferences.
In my classroom I have a fluency binder. I print two copies of each story and put them in a clear paper protector. When the student reads to me they have one and I use the other one to mark errors with a dry erase marker. I print the tracker sheets front to back, I print enough for my whole class. l hole punch them and put them in the binder with the stories.
When you are ready to test you grab the binder and you have everything you need.
