Calling all secondary teachers—let’s get organized! Keeping a student information and documentation binder is a great way to stay organized and quickly locate important information when necessary. Instruct students to fill out the “Student Information Page” on the first day of school or early in the year. You can then organize the pages by class period or alphabetically in your information binder(s). Some teachers prefer to have one large binder and use dividers to separate class periods or an individual binder for each class period. Use the method that works best for you!
The “Parent Contact Log” can be photocopied on the back side of the “Student Information Page” so you have student information and documentation all in one place.
The “Student Information Page” has spaces for student name and nickname, student ID #, student email, contact information for parents/guardians, class schedule, extra-curricular activities and interests. The “Parent Contact Log” page has a documentation chart to keep track of parental contact dates, method of contact (email, phone, face-to-face conference), reason for contact, and outcome of contact. There is additional space to make other documentation notes.
**Although this was made for secondary teachers, elementary teachers are welcome to purchase and use! However, younger students most likely won’t have class “periods,” know their student ID numbers, or know their parent’s contact information off hand. You can, however, send the form home and have the parents fill out the information sheet.
