Need a quick way to record your observations of seizures?

Many of my students have multiple disabilities and have seizures daily. I need a quick way to record what occurred during their seizure, how long it was, O2 levels, etc. But I can't just sit there and write a narrative!

*THESE SHEETS ARE FOR YOU! For much of the data here - you just circle the observation and you're on your way. These sheets come in 2 forms: with and without O2 levels.

Data is based on important characteristics that the National Epilepsy Foundation looks for during a seizure.
