This packet is designed to be completed with a teacher’s assistance. It should be approached with sensitivity since you will be discussing a person’s disability with them. It is important for people with disabilities to be aware of their disability and their needs so they can successfully advocate for themselves.

Some of the pages may not be appropriate for all students. The packet is designed to be customized for each individual; you may include as many pages as needed. The pages are designed for students but can also be used for adults.
