A proven-to-work self-monitoring sheet for middle and high school students to use to monitor their own behavior. Used with students with IEP's and/ or moderate to severe behavior issues. Two versions are available for students to understand simple self- monitoring language and move on to monitor more precise behavior.
Provide student with one of the two versions of the monitoring sheet at the beginning of class. Explain to student it is up to them to fill it out based on their own behavior by the end of the class time.
