This activity is excellent for students to practice self-monitoring their attention at school. The sheet gives instructions as to what "on-task" behavior looks like, you begin a timer that goes off every 5 to 10 minutes (many can be easily found on YouTube) and at each ring, students must mark down if they were on task or not.
After several periods or days of tracking (there are some blanks so you can choose what time slot in the day you want to practice this skill), a blank graph at the bottom allows students to view their trajectory.
The sheet could be integrated with work on mathematics such as ordered pairs and graphing, or done together as a "we-do" activity for younger students. Even as an adult I was shocked when I practiced this sheet during a meeting! Really brought my self-awareness back to keeping attention on the task at hand.
